Perkins, who starts in the role next month, will lead W's five-strong creative team, reporting to agency founder Warren Johnson.
Perkins joined MHP as creative director in 2013. His work for the agency includes award-winning campaigns Missing Type for the NHS and Christmas Tinner for games retailer Game.
Prior to MHP, Perkins was deputy managing director at Cow PR between 2007 and 2012. Before this he was associate director at Frank.
According to Johnson, the hire is "an important step" in nurturing the agency's existing talent, increasing its creative firepower and for "winning more awards".
W has been shortlisted in four categories at this year's PRWeek Awards, including for Mid-Sized Consultancy of the Year. It ranked 52nd in the PRWeek UK Top 150 for 2017.
Johnson said: "Mark and I began our careers together [at The Red Consultancy] 20 years ago, and have remained great friends ever since. I've admired his creative career and ambition from afar, so it's fantastic to be reunited - hopefully with fewer hangovers than in the nineties."
On his appointment, Perkins said: "My ambition is to help create industry-leading and disruptive campaigns. Work that raises the bar, creates fame and blurs the traditional boundaries between PR and advertising. I can't wait to get started."
